Additional contact us fields

I have installed this Mod and turned the phone number Field Status to ON but I do not see anything on my Contact Us page. Why?

Very vague report. Please state what you did - A to Z.

I followed the instuctions on the install note;

1-I loaded all the files including:
- includes/modules/<your_template>/pages/contact_us/header_php.php.
- includes/templates/<your_template>/templates/tpl_contact_us_default.php

I am not sure if I need to make any modifications to these 2 files?

2-I then installed the SQL patch in the admin

3-Turned on the MOD in the admin - customers -> Add Contact us fields

It shows up in the admin but not on my site.

As mentionned from my installation text file - it is required.

And did you edited your contact us template in order to be shown ? Templates must be edited for each fields you create. Examples are included in my package on how to proceed for each fields.

Like I said, for newbies, examples are included from my package. Simply use a comparing tool (such as: Winmerge or Beyond Compare 2) to see how this works.

Which are the examples in your package?

I have installed Beyond Compare 2 and compared these 2 files;

I have no idea wha to do with this program and these file? You said "see how this works" see how what works?

Inside that file, you should see an uncommented title like: // Additional contact us fields until a // End of additional contact us fields. That block represents the example.

When creating your fields from the admin, you must add each of these fields under your contact us template file as their definition names under your language's extra_definition folder (which I also included the file on the package).

When I tried this my contact us page went blank.

With this MOD shouldn't the "phone number" field you have included be fully functional with the installation. If not why?

Each fields you will create should function correctly once you have also initiate it under your extra_definitions folder + your contact template file (as shown from my example). The phone number field should appear on the template if you have compared my readings with yours correctly. However, you must also assure that the physical field does exist under your listing table. Otherwise, it is obvious that it won't work.
