Additional contact us fields

So what happened to this mod?
Why did TheOracle remove it?
Did it not work?
If it did work, can someone else supply a link to it?
Did it support 'non-required' fields?

I could really use some extra field in the contact form. I've tried Ryk's suggestion on how to do it, but that doesn't support the non-required fields.

is there a way to have a drop down menu for state and country like the registration page for zen cart. and put in the contact us page.

is there a way to make the drop menu in the contact us from this page on zen cart contact page.

I have done the steps from theorcale and from ryk. I think Ryk for contact us addition was better. But now i need to know how to make a drop down menu for state. I have actually got the country by copying and pasting from the registration page code. but when i tried to do the same with the city it doesnt work. so right now i only got the country down. so any help would be appreciated thank you.
