Would having IPN and Express enabled at the same time do this?

It would really help if you were to explain more information about your store.
Like:
- What version of Zen Cart you're using
- What addons/contributions you've installed
- What Zen Cart patches you've applied
- What version of PHP you're using
- URL to your site
etc.
Maybe some of these will help -- just shooting in the dark because I'm not sure what you're running on your site.

I'm running ZenCart1.3.7.1 and the DataBase patch level is the same.
The PHP Version is 4.4.7
No Zen Cart patches have been applied
I have installed The Silver fish template

Can you make the problem happen yourself when you do a test transaction?
You should turn on debug-logging in order to capture what's happening in the background while the customer is attempting checkout: http://www.zen-cart.com/forum/showthread.php?t=61199.
