Adding Attribute with Price Attached

Hello,

So what I don't know how to do but that I'm wanting to do is the following:

I am wanting to add an attribute as a checkbox that if checked will add $1.99 to the product price when added to the cart.

I'm also wanting the layout to look as such:

Title of Checkbox *checkbox*

Does anyone know how to achieve this goal? I am kind of lost cause whenever I add a checkbox the lay out looks like this

Option Name *checkbox* Option Value

Which is the problem and I also don't know how to add a price to an attribute. So if anyone can give me instructions on how to do this I would greatly appreciate it.

I don't know of any (easy) way to remove the attribute value text, but you set your attribute prices by adding the attribute value to your product(s) via the Attribute Manager.
