How do I add additional fields to the "Contact Us" page. Right now it only asks for the Name and email.
I would like to ask for the:
"Title", "Address", "Phone" etc etc.

Is there anything out there that will add the Name, Title, Address, Email and Phone / Fax number to the "Contact Us" page?
I have already received inquiries from the contact us form asking me to call them but they do not leave a phone number.
