Need *URGENT* HELP please!! Fatal Error order_total.php

When trying to progress to Step 2 of my store checkout, I'm getting the following message:

Fatal error: Cannot instantiate non-existent class: ot_insurance in /home3/mindy/kings-kreations-www/zen/includes/classes/order_total.php on line 36

Here is line 36: $GLOBALS[$class] = new $class;

I had been working on making some changes to my shipping insurance module tonight. I wasn't even working in the area that is giving me the error message. I re-uploaded the order_total.php file from my backup disc made about a week ago. No change to the error. I have not made any other changes or upgrades. I am using version 1.3.7. My site is www.kings-kreations.com/zen.

Can someone PLEASE help?!?!

Common causes of "class notifier not being loaded" and "can't find autoload_func" are:
1. Files weren't uploaded properly for the /includes folder. Perhaps some were missed, or incomplete, etc.
2. Files weren't uploaded to the right location, namely also the /includes folder.
3. Permissions set to tight, thereby preventing access to files.

Do you have a file named something like ot_insurance?

D'oh...I'm so embarrassed I even had to ask!

There are two ot_insurance.php files & I uploaded one to the wrong place. Thanks for your help!

Not an issue!!

For future reference you looked at the file order_total.php referenced and most always this error is the result from a previous glitch and for this it was(highlighted):

Fatal error: Cannot instantiate non-existent class: ot_insurance in /home3/mindy/kings-kreations-www/zen/includes/classes/order_total.php on line 36

For the most part always look to the initial bump and not to the subsequent resulting file errors
