The only way this can be researched further is by using your account/site for testing. Please PM me if you're willing to allow me access to your site to run some diagnostics with the goal of finding a fix.
I've tried replicating this situation on my own test servers and cannot. So will need your environment in order to find the cause and the fix.
Naturally, will do it in a way that is unobtrusive to live transactions.
In the meantime, you can disable the hash-check by making a brief change in the code, as highlighted here:
(although, (this said for future readers of this post) if you're using the hash-check for security, disabling the check is NOT WISE)
