The main problem here, seems to be one of shipping costs. Paypal already has the card holders name and address and details of the items they are purchasing.
Apart from incorporating a zone or country drop down on the cart page, there is only one practical way of giving shipping information to PayPal, and that is to add it as options to the product description. i.e. Shipping options 'Own Country x amount' 'Surrounding countries plus x amount' 'other countries x amount'. That facility already exists in Zen Cart.
Now if you had a PayPal IPN quick checkout, where the buyer went straight to a shipping options page instead, then clicked to take them straight to PayPal, you would have a radically streamlined checkout setup.
My philosophy is the quicker a buyer can pay for their item, the quicker they can get on with their lives. Anything that puts obstacles in the path of an impulse buyer, can only ultimately cost us all potential sales.
