Affiliate Module

I am looking for a good Affiliate Module/Add-on for Zen-Cart
with the following features:

* Can calculate precentage of order without the shipping cost thus on the actual products sold only.

* Affiliates can get payed out through PayPal or Bank Transaction,

* I can customise (add new,delete,edit) fields required by the Affiliate Registration

* I can customise (add new,delete,edit) fields required by the Payment Settings Page
