You are not getting ez-page links in header, footer or sidebox because you have their sort orders set to 0. Set them to actual numbers and they will show up.
Your previous posts gave the impression that you were not getting the TOC at all. If I had known that you were getting an incomplete TOC, I would have known to direct you to this thread:I am not getting a listing of the Table of Contents
