Is there a way to add lens type into attribute or any other mod as below?

That is by creating one option name and them associating the 5 option values to it

That will require a good bit of custom code/ moving the attribute selection to another page

Giving a bit mor thought to this it is akin to dependent attributes which ZenCart has no support for

Meaning some attributes are displayed on the product page and "depemding" upon the selection in that lens one you either go to the cart or to another page

Adding dependent attributes will be complicated

It will be significant custom javascript coding, not suitable for attempting in a forum post. You will either need to code it yourself or hire someone who is good at javascript and Zen Cart to do it for you.
