SEO URL rewriter issue

I'm kind of hoping someone else who has had this problem will chime in and describe how it happens. I haven't yet had time to dup it; I just saw it yesterday. Obviously I am going to try.

Just started receiving this error after installing CEON URI Mapping. Merged files as suggested and followed install instructions correctly. Changed the URI in mens clothing category and it saved fine. When I try to tick the auto generate box and click save I receive a Not Found error in Admin for this page - categories.php?action=update_category&cPath=

When I check Logs I receive this error:

Going to have to remove. Can anyone recommend a different seo rewriter which works with 155f?

What php version is being used on the system of the original post? The majority of changes implemented by webchills were specific to php 7.x. The software does work on ZC 1.5.5f whether that downloaded from the Zen Cart site or from github say from Torvista.
