Only thing that comes to mind is that the customer was there when the site was not showcase and did a reorder.

the site has been in showcase mode for several weeks now. No, he had not ordered that product before but had ordered before.
So the question remains: why would the shopping cart appear for him to check out when my cart was cleared of products. The issue looks like in that page - I was worrying about the template til I realized this as the shopping cart has not been altered in core or in the template file. The template has no core overrides and the site was updated to 1.5.7d last April.
Other thoughts... Legacy link from google?
What did the customer relate as to how they got the product into a cart?

He had put the product into the cart some time ago. He must have had the login link saved as a bookmark. That's what I did when I logged in - just go to the login link. The template has the header link for the shopping cart. I saw I had items in the cart and click that link and those items disappeared in front of my eyes. I assume that's how it's supposed to work.
I did go ahead and cleared the customer basket table which cures the problem.
I have to assume that normal testing for this probably didn't include carts with thousands of active customers. These guys have sold the business so this is truly an abnormal situation.
Consider yourself lucky. One of our customers decided (before us) to reuse IDs. As the product became unavailable, they just replaced the info with a new item's information!
Had them for a couple of years and still getting the occasional glitch. Especially with SBA.Over 66k customers and no telling how may IDs.

Stay calm, the problems were not with SBA but that the owner did not remove the SBA entries in the DB when they reused the ID numbers. It's been months since a warning showed up that an attribute was not found for ID ##.

No problem. Had added a display to try to catch issues while using the sba admin. Continue to look for ways causing or finding anomalies. Repurposing product id's certainly could cause issues especially if changes are made specifically to the database without proper coordination of action.
