Yes, that's probably true, I had problems with my previous host and so took the opportunity to upgrade, but there were still issues with their server which they failed to fix in time, I migrated to a better host and was up and running before the original host got off their butts.

Can't repro, no idea how this happened. You can find the SQL for creating the customer_groups table in mysql_zencart.sql in zc_install/sql/install.

Both tables are missing in my case (upgrading from 1.3.9 to 1.5.8):

customer_groups;
customers_to_groups;

"Customer Groups" works fine now after manually adding these two tables.
