It's not that Important as the fields are self explainitory.
I can't login in in any case You have the US locked out in your "select country" field.

I'm not sure what the problem is but I've got it working on three of my own sites and I've checked a couple of other sites from this thread and and it seems to be working.

You should have the following folders and files:

includes/languages/english/YOUR_TEMPLATE/popup_links_help.php
includes/modules/pages/popup_links_help/header_php.php
includes/modules/pages/popup_links_help/jscript_main.php
includes/templates/YOUR_TEMPLATE/popup_links_helptpl_main_page.php
