I have used your module for quite some time. Suddenly, it is no where to be found on my site at www.bobkatgifts.net . The ver is 3.11 - it is a no brainer to install - only two files. Appreciate your help.

I had the same issue only if was giving that error for virtual products which didn't require shipping. Working fine now.

I went back to 3.10 version - have no idea at all why 3.11 would not display. To bad - I liked the extra features.

Could you tell me how to make the checkbox "checked" by default please?

I mean I want to offer insurance by default to my customer so if they don't want it they have to uncheck it

Has anyone had this problem..... I am using PayPal Website Payments Pro - works great. However, you are required to have PayPal express also installed. If a customer choses PayPal express - they are sent straight to the PayPal site and they do not see and therefore are not given the choice of getting insurance or not. Is this a problem with the program or am I missing something. Thanks
