So, I don't know what it is that causes this, but is there a way to get rid of it?

The pages load slow, and once a user gets frustrated and clicks again, s/he gets this error.

What's a sysadmin to do?

I don't know about this error for db_catche table . this is usually caused by not auto incrementing but db_catche has no auto increment filed to begin with ..

you may want to turn off the pars time , query time and number of queries from your site , that might help to speed up your site a bit .

was not able to reproduce the error you mentioned.

thanks for checking. I get it very rarely anymore (I feel like some of my shared hosting neighbours moved out!) but it comes up at the worst times like when I'm trying to show something to my boss and inevitabley it comes up right then and there. /wrists

I got the same error today . my problem was , mysql server was offline .. once i told me host about the problem , they fixed it , but i was keep clicking on refresh button.
once mysql fully started , i got this error .. but then everything back to normal .

it chances are that ,your mysql server is haveally loaded and there is delay in response and clicking on links multiple time might generate duplicates.. other then that , it shouldn't be a problem.
