IMO there are only two versions of IH2 that you should consider installing:

Hopefully one of these versions is the one you are now using. They are stable and should install pretty much "out the box". The latest version (Ver 2.0 rev 7) is a meaner leaner version that removes some of the "leftover" files from the original release of IH2 that are totally not needed for IH2 to run. (For example the "shoppad" folder -- turns out you can delete that folder in it's entirety from ALL prior versions). The readme is much improved (if I do say so myself!) and clarifies the app and it's purpose a little more clearly. Ver 2.0 rev 7 also includes some of the improvements that have been posted here in this thread by community contributors including:
Ver 2.0 rev 7 of IH2 is based on the Jan 16 2009 version of IH2 authored by yellow1912. So with regards to the bolded part of your post (above), my suggestion is that regardless of whatever beef you and ckosloff have, Ver 2.0 rev 7 is a good version for you to install and you should give it some consideration..
- The improved version of bmz_image_handler.class.php
- An improved version of jscript_imagehover.js.
The problematic version of IH2 IMO is this one:

In short this version contains the now INFAMOUS "Optional Extras" folder. IMO, the purpose of these files was not very well documented. The purpose of these files as it turns out was to modify the default behavior of the main product image.
However, in testing, some of us discovered that this "new" functionality disabled some lightbox contributions functionality (specifically the Zen Lightbox and Fual Lightbox add-ons). In short the "Optional Extras" files would prevent your lightbox from working with the main product image.
Sure there's language in the documentation for that version that seems to outline all the "fixes", but it's not necessarily written in "laymen" terms. Plus the documentation it fails to notify the user that these changes will likely "break" your lightbox contributions. (this is likely because the author did not use or even test his "fixes" with any lightbox contribution)
