Contact us Page Broken

Hello, Site has been working great for a year.

Now today I check and my contact us page and it only PARTIALLY loads and says "This Is The Server Information".....

Use FTP and check your file dates modified
If there has been any change it will bear a recent gate

I havent done anything technical to the site lately... just the usual add some new products and upload new photos. Any ideas on where to start??

I would try uploading a fresh copy of includes/templates/template_default/templates/tpl_contact_us_default.php to your override folder (includes/templates/az/templates/tpl_contact_us_default.php). That file has been altered by someone, looking at the page source.

Search for that phrase (this is the server information) in Admin - Tools - Developers Tool Kit. That will tell you what file has been edited. Also look at define_contact_us.php in the Define Pages Editor.
