It isn't logical, but it's true. I'm no PHP wizard, but I looked at every file in the SBA mod for 1.5, and I couldn't see anything that would affect additional images (though I know nothing about jquery).
Nevertheless, I installed a fresh copy of 1.5.0 and SBA for 1.5, no other mods or changes, and the additional images in the demo products disappeared.
I just installed the SBA 1.5 a few days ago and I'm pretty sure that my additional images worked before I installed it but they aren't working now.
I installed the SBA 1.5beta on my test site first to make sure it worked but I didn't have pictures on my test product. I went back to my test product on my test site and added a picture and uploaded an additional image. The additional image doesn't show up there either.

I transferred an old site to a new. There were multi-pics in the old one.
They work, and I can add additional pics to the working multi-pic pages.
If I add a NEW product, I CANNOT add multi-pics. They just don't show up...
It's a very customized site, but why will it work with what WAS there, but I cant add new?

When I couldn't get multiple images to display for my items, it was because I only had the large images and no smaller versions.
This was my solution:
I made large and small versions of each image and then I uploaded those images to the appropriate folders after naming them.
The "images" folder now contains myproduct.jpg, myproduct_2.jpg and myproduct_3.jpg.
The subfolder named "large" now contains myproduct_2_LRG.jpg and myproduct_3_LRG.jpg.
The subfolder named "medium" only contains one medium-sized version of the first image, myproduct_MED.jpg.

And the reason you had the issue with additional images is because you had no images in the "default" folder. (the "images" folder) The way Zen Cart "recognizes" additional images requires that a "default" image must exist. (it then matches the additional images based on the NAME of the default image)
For anyone who doesn't want to HAVE to create three different sized images, you CAN simply load your largest image into the "default" ("images") folder and Zen Cart will take care of displaying the proper sizes in your store based on your Configuration > Images settings.
The purpose of the medium and large folders is if you want to create three distinct image sizes yourself OR if you want the medium or large images to be different images than your thumbnail image.
