Sorry, I'm still confused/unsure.
What were the filenames AFTER you uploaded them to DB, but BEFORE renaming them?
That's the piece of the puzzle of interest/importance.
The "export" (which I assume to mean downloading from Dropbox) isn't a surprise/puzzle/mystery, because that would be the normal/expected thing anyway.
The renaming being irrelevant.
<still scratching head trying to figure out the mystery of the iPad apparently giving all images the same name>

On the ipad itself we couldn't see names / properties. To be honest I didn't upload any images today to the db that weren't renamed.
When we uploaded them to drop box they had numerical names eg: 2151548784245.jpg

That looks like it is probably some kind of timestamp.
Shall we now revisit the opening post?
"They tell me that all the images on the Ipad are called image.jpg"
To quote Dr House (remember that show?) "Everyone Lies" (especially customers and patients) ;-)
"Apparently you can't rename an image file on the Ipad"
I've googled this, it appears partially correct in that apparently it requires 3rd party software. However, it can be done.
"So when they upload their multiple images it's just replacing images instead of showing multiple images"
I am going to assume that this is an assumption, otherwise the same would apply to the uploads to Dropbox.
"I told them maybe they need to export to a pc, rename them, then import them back with different names.
Is there a way around this?"
It depends on which "problem" is now to be solved.
The export, rename, import method can be solved with a 3rd party iPad app.
The problem of all images having the same name and therefore overwriting each other doesn't appear to exist, at least based on the findings of uploading the files to Dropbox.
Anything else?

Re: Apple Ipad and uploading multiple images to the store
So everytime I receive an image via text from an idevice it comes across as FullImage.jpg. If I receive it by email it comes across as image.JPG. (Notice the capitalized extension).
IH4 would be a way to simplify the naming issue, third party software, and would provide the ability to present images in varying sizes amenable to devices. Of course that said, the typical resolution of such an image may be an issue if it is not already addressed in the latest version of IH4...
