Sorry - the site with the problem is www.daisywest.co.uk - there are other sites on the same server where the Image Handler is functioning correctly.

So what was the issue?? It might help others if you post it here.. Like for example I discovered that additional images didn't work on all products and the issue was my client was using single quotes in her product names.. It's not an Image Handler issue, but it was good information to report to others (who might have thought it was an Image Handler problem like I did)..
OK, the issue was that another contribution - Ultimate SEO URLS - had been installed which overwrote the zen_image function in html_output.html. This caused the images processing override to prevent Image Handler from running.
Simply really - but I got sidetracked in looking at IH rather than the underlying issue!

It make it so you can hover over the "medium" or product image and it will pop up with the large one. It works great!
I made a small change which is noted in the readme file, there was a missing <br /> that causes the "zoom" text to sit next to the image instead of under it if the "medium zoom" box is turned off in admin.
The other note of caution with this is patch is, if you have made the suggested changes listed in the security patches to the includes/functions/html_output.php file, do NOT directly upload that file from this patch, but instead do a compare and merge them or you will undo your previous changes.![]()
All the other files can just be uploaded as-is once you change the YOUR_TEMPLATE file name to your actual file name.
