Easy Populate error on Download Complete .csv

I have tried searching the forum, no luck. I get the following error msg when trying to download complete .csv file

What VERSION of EP?

These errors indicate a bad file, and/or installation to an incorrect location.

Many reports of this version being "shaky".

Despite its "age", 1.2.5.4 is still the most reliable - even though it uses TAB DELIMITED format instead of CSV.

Consider removing 1.2.5.6 and loading 1.2.5.4

I have 1.2.5.6 on another site and works great, I would prefer not to start mixing versions.

Try resending (via FTP) the following file:

admin/easypopulate.php

Overwrite the file that's there.

I tried that, no luck so I removed all the files. except header_navigation.php, not sure on removal so I renamed it for now. Also under my admin/configuration tab I have 3 Easy Populate links?
