Unfortunately I do not know what could be causing the issue. It's not caused by IH2 (which I was fairly certain of already) but clearly there is something in your template which is limiting the number of additional images.. (I am pretty confident that this is the root cause of your issue) If it is a template you got from the downloads section, then you can try posing a note in the support thread for the template, but I think you'll wanna try posting a new thread in the layout section of the forum..

Without having the benefit of seeing your store code I would guess that your template somehow modifies the additional images code in a way which limits the display to only 3 images..
very bizarre as it was working before latest zen update i dunno i will work on it, i dont know how to find out in the template whats causing it if it is the template which i don't think it is. Now that i re-activated IH2 the 3 images are gone ughhh
