how to include text and image with text attribute?

hi, im trying to create an effect similar to what you see here, for entering clothes measurement attributes: http://www.makeyourownjeans.com/index.php?main_page=product_info&cPath=1&products_id=33

How do I include explanatory measurement text and images like they do? I used test attributes for waist size. And in attribute controller chore the linage, and tried inputting test in the comments field,but was restricted to only 1 or 2 lines.
Also under the field where the person would input their measurement, TEXT appears for some reason.
Also if you look at this page, how do they include the "step 1 "image before each attribute? : http://empiretailors.com/catalog/product_info.php?products_id=67

They could either be using an additional piece of software to achieve that effect, or had someone write some code for their pages.

There's a mod in the add on section that will do something similar
