Odd title bar and title page behavior

I noticed on several of my pages the title bar title and page title description is reading "Contact Us". This includes the home page, all products, new products, login/register, basically all pages (excluding) the main product category navigation pages and product subcategories pages.

Now I did recently install and remove the:
captcha-number-subject-combo-in-one-1.0 mod.

Any help finding the villain would be great. Thanks

Since we don't have a crystal ball handy, how's about a peek at your site?
