Newsletter Subscribe error. Can anyone help please?

I am getting the following message on my subscribe to newsletter page - can anyone help please?

I can't find any files that look as if they might be the file it is referring to and am really struggling here. Any help gratefully received!

Well, maybe that's the problem. The file is missing. Check your original unzipped folders and FTP up the missing file.
