special date box in checkout pages

Hello i'm having a problem creating another small "comment" box on the checkout pages. I hope someone can help me with this issue my website is about seling flower arrangements and instead of putting a calendar i just want to put a small box in which people can just put the date of delivery because usually in flowers websites there are three small boxes one to write Category; to write birthday or anniversary..eg; Date of delivery and Comment box to put card message and signature. If somebody did this please can help me out to solve this inconvenient. I thank you so much in advance.:mellow:

Attributes! Use text option. Create one for delivery date, one for message, and one for occassion. You'll be ready to roll!

ok thank you very much lady but i just need some guide on this one i go to configuration the i choose attribute settings? well then i don't see text only i mean i'm checking all those tex-pricing ,..eg
