Sale Final but items stayed in the cart?

I have been using ZenCart since 2005 and this instance has only happened maybe 5 or 6 times ever. I have not had this problem in a long time but it has happened twice in the last month and I think one more time a couple of months before. I am afraid that it is becoming more common. I process about 150 orders a month without problem so that is what confuses me.

The customer checks out with the "Authorize.Net SIM" module and I successfully receive the payment but the items remain in the cart as if the card was declined and the sale was not complete. I think that it has even happened with the "Paypal Express Checkout" module once. I have up until this point just thought it was some kind of internet glitch because it was so rare but now I am a little worried.

I am running 1.3.7

Any help and or suggestions will be greatly appreciated.

because it is such a rare occurrence, the first thing that springs to mind is that it may be just a browser (cache) issue.

Maybe worth a thought?

My Google payment module does that for every sale. Hell, I thought that was normal behavior for off-site payments. Maybe it is.

I did ask about it before but no one could answer.
