How to set up country zones for flat rate shipping?

If you have bad data, then that is something you need to fix to verify that the default is a valid address book entry ...

You might try testing your database tables to see if the:
customers
address_book

have matching records for the customers field:
customers_default_address_id

and the address_book on:
address_book_id
