the logo image file is identified in includes/languages/english/header.php

define('HEADER_LOGO_WIDTH', '192px');
 define('HEADER_LOGO_HEIGHT', '64px');
 define('HEADER_LOGO_IMAGE', 'logo.gif');

if you edit the file, be sure to save it in your override folder includes/languages/english/YOUR_TEMPLATE/header.php

The image file itself should be located in the includes/templates/YOUR_TEMPLATE/images folder

None of them. It's an image file.

Which image file is defined in the languages/YOUR_LANGUAGE/YOUR_TEMPLATE/header.php file. This information is then used in the templates/common/YOUR_TEMPLATE/tpl_header.php template.
