Creating a question box at checkout

I am not sure what it is called, but I want to make a little box at checkout that the customers can check if they want information pamphlet added in with their order. It would be similiar to the newsletter subscribe, but it would not subscribe them up to anything.
Any ideas on how to make this???

Also how do I make it so that html is the chosen newsletter choice, and they can then change it to text if they want?

Check in here I know I have seen several add ons for changing the login and checkout options.... This one (http://www.zen-cart.com/index.php?main_page=product_contrib_info&products_id=845) has a dropdown on checkout for whatever you like.

The setting for default choice is Customer Default Email Preference in Configuration >>> Customer Details
