Need Feedback form on Checkout Confirmation page

We are looking to put a short feedback form on the checkout confirmation page, just before the "confirm order" button.

Thing is - the basic HTML form I put up doesn't do the trick because when you submit the form, it goes to the thank you page.

Now, this prevents one from clicking the "confirm order" button.

However, the order does show up in the Admin AND the customer receives order confirmation. So, I assume one didn't need to click on the "confirm order" button ???

What do you think?

Just wanted to come back and say that after much thinking, I concluded that I couldn't do this form on the checkout confirmation page because of PayPal as a payment option. The customer couldn't click on submit for the survey form AND go to PayPal to pay at the same time. So, I just put the form on the success page.

I can think of two ways to do this. A small popup form. That will keep them on the same page. Or an iframe and have the form in the iframe with a response. Again they would be on the same page.
