My Customer Charged Tax Twice

I have just made a sale and my customer was charged 16% instead of 8%. And at the bottom of the invoice it shows ON RST + ON RST.

Just wondering if someone could help me out.

My customer was charged tax twice on the same purchase. On the invoice at the tax line it has ONT RST + ONT RST.

Just wondering if anyone has any ideas as to why this may have occured.
