captcha-combo-in-one-1.0 won't display image

Just instaleed the: captcha-number-subject-combo-in-one-1.0, everything looks good accept instead of an image I get "enable this picture" text. Is it an SQL issue? if so how do I reinstall the SQL? anybody using this mudual? Thanks for helping frustrated individual.

Figure it out...missed the include files...my bad.
