adding of some new fields in order table

I want to add some new field in order table. I've altered in order table defined new fields.

Here I want when a user placing an order so on the checkout shipping page i want to add some new fields like some questioner in yes/no (radio button) format like - Do you smoke? How is your blood pressure? Do you have any allergies? (including medications) etc. here i should mention that i am working on a medical drug project.

So can anyone guide me how can I do this so that after confirm the order table this data will also store in order table with the all order data.
