Can't add new administrator

I'm using v1.38a and am unable to add a new administrator for it. When click insert and add the appropriate information and submit the form it just redirects to the admin home page. EVERYTHING else appears to be functioning correctly. I've re-uploaded all files and re-imported the database and still have the same issue.

What would cause me to not be able to add a new administrator?

Sounds like a symptom of having the most recent security patch only partially installed. I'd check you admin/includes/functions/html_output.php file. Sounds like it's not been properly updated.
