Re: Form on homepage with radio buttons?

I'm not exactly sure what you want to do. You want to add a second Contact Us page, containing buttons linking to your Zencart main page? Could you explain in a little more detail?

gjh42:

He wants the contact form right on the home page.

Using includes/languages/english/html_includes/define_main_page.php

couldn't he just use a php require statement?

<?php require($template->get_template_dir('tpl_contact_us_default.php',DIR_WS_TEMPLATE, $current_page_base,'templates'). '/tpl_contact_us_default.php'); ?>
