Unable to load store

Zen Cart Version - Zen Cart 1.3.8a
No upgrades
No addons

I am unable to get my cart to show up.

I had the site up a few months ago and I switched hosting. I have come back to GoDaddy. I think the index.php file is not correct. It is loading the under construction splash page and not the store. What should the index.php file contain in order for the cart to load?

NO EDITS to the index.php are required

You show the link to the domain root, but it redirects to monsterfishusa.com/monstercart/. Where did you install your Zen Cart files?

Zen cart files are installed as shown in the attached pictures.
There are two pictures attached of the file structure. Any help is greatly appreciated.

The next question is where the configure files think they are located. Did you edit them to match the current folder structure?
Post your /includes/configure.php with db/user/password blanked out.
