Captcha Image Issue - Enable This Picture

I have just installed "captcha-number-subject-combo-in-one-1.0" addon (http://www.zen-cart.com/index.php?main_page=product_contrib_info&products_id=915)

It added the extra fields Ok but the captcha image is not appearing. In its place it says - enable this picture.

Any help would be great

I know what I did wrong, I did not see the file "captcha_ing.php" in the root directory.

Once I uploaded this file to the root directory of my site it worked perfectly.
