Need to add doctor to customer details

Hi,

We have recently been obtaining orders from doctors. Due to a conflict their cards, i really need to have an additional check field for Doctor on the customer details page. I looked into doing this myself however as it is currently a boolean value im at a loss of how to edit this.

If anyone has any insight as to how to achieve this result i would be very appreciative.
