how to add multiple fields to contact us page

Is there an easy way to add multiple fields to the contact us page like I have here:

http://cogliaticatering.com/contact.htm

Or should I use ezpages and just set it up the way it is with formmail. Probably not a good idea as the correspondence will not be stored in the database.

Or maybe there is a mod/addon that will do this easily?
