Repaired hacker damage - new customer form still broken

Repaired hacker damage - new customer form still broken

zen cart 1.3.8a
My store was hacked as is explained on this page:

I have repaired the damage as outlined on that page but the new customer info form is still blank:
http://www.catmtn.com/tyg-online/

I have narrowed down the problem to an issue with the database by setting up another new store and pointing it at the old database. The result is the same; a blank and broken new customer form.

Any ideas how I can fix this?

**Admin - Configuration - My Store - Store Name ** Remove all the garbage and reinsert your actual store name.
