Adding option comments to cart and order confirmation email

I need to add option comments displayed with the product options to an option listed in the cart and on the order confirmation.
I tried outputing the $product array to find it. I have trawled around the files to find a way to do this, but can't.

Any pointers would be much appreciated.

Using 1.3.8
