Flat Rate Per Item Shipping ?

I have a client who wants to have a flat rate per item shipping method.
There will be a $10 fee for the first 1 to 10 items ordered, but they want $1 added to each and every item over 10.

Example:
Ordered 1 to 10 = $10
Ordered 11 = $11
Ordered 12 = $12
etc, etc,

Use the Item Rate item shipping module and customize it to check the count ...

Set the rate to 1 and and the Handling to 9 then customize the file to change the count used to reset the number of items used when <= 10 to be 1 ...
