So when we issue a refund, it updates in paypal and in the bottom of the order you see the refund has been completed. However, it does not update in the main order info? Is there a way to get it to do this?

Far as I know, Zencart doesn't change order info when refunds are made.

Correct. In Zen Cart v1.x, the final total will not be adjusted when issuing refunds. The comments section is used to show you what changes you have made to the order.

Yes, it's part of the order comment history, visible in their My Account area.
