Help needed - Address areas State/County text area / drop down

I've set-up a site for a friend and seem to have broken all address input sections/forms when selecting the UK, after spending all day on forums and google I'm no further along and need to ask for help.

I'm pretty sure this has something to-do with adding all the UK counties to my database but I have no idea why.

now if anyone tries to add an address or create an account they see an input box for the county/state and the default of UK for the country. not a drop down box, if you type in the box and click submit there is an error and the drop box appears with all UK counties. I thought the java script should pre-populate the drop-downs and show them for all country's that have states and a text input for the ones without, can someone please help I'm going round in circles trying to find the problem.

if it helps anyone I can post an attachment of the different parts of my DB for the zone/countries etc from my DB.

this was user error on my part, i had not clicked the show drop down option under the admin section. now for the countries with states/counties i get two drop downs one saying please type below and the other with the state drop downs. I feel this is a little messy and would be better if the drop down that is labelled type below was a read only text input or better yet just text, but I'm not very good at java if anyone else has changed the way this looks please let me know.
