Contact us page question

The contact us page that comes with Zen Cart has a built in contact form with the fields name/address/message. Is it possible to add more fields to this contact form? ideally phone number? If so where or what are my options?

i got this little issue with my contact us form the phone number text is not there it just has this

reinstalled it now it's working

Hi everyone,

I have just installed this mod and when I try to submit the message if I leave any other fields empty including the phone number field on the contact us page. It comes up with a message saying to enter in a phone number as well as any other fields I left empty.

However if I fill in all the other fields but leave phone number field still empty I get no warning message to enter in my phone number and the form is submitted.

Anyone know why the warning isn't popping back up to tell me to fill in my phone number before submitting the form?

I have it installed on v1.3.9h.
