custom meta tags for defined pages

where do you set custom meta tag info for defined pages (title, keywords, description, etc)?

FYI: the extra PAGE in the 2 examples was preventing the info from displaying. Remove it. Thanks Linda. :-)

btw LEATHERMAN_TOOL is the end of the URL, as this is my keyword i'm using it for des, title n keyword too.
