How do I add a comment form before the submit order form?

The lady I'm helping is selling a bracelet with a charm of their choice. I think the only easy way for this is to create a small comment field to put down which charm they want. Where and how do I create this feild? Thanks for your help!

Sorry I wasn't clear on the subject!! I wanted a comment field form so She know which charm to give for free (I know there's a free price option) but way too complicated for her to update all the time

She rather have someone just purchase a "Bracelet Kit" and then a comment field telling her which charm they want.

ps. Is there anyway to add a Icon Item to the shopping page and when they click on it, they can add a note to it? and then submit it to the shopping cart?

Unless I stll don't understand, I still think you want to use attributes.

You can set an attribute as a text input field where the customer can enter the name of the charm.

Adding custom fields to various pages in Zencart and have the information entered in them display all the way through the process is a pretty advanced coding job.
